SayNation
For surveys I've been using a site called SayNation. You get points for surveys (Normally around 1,000 - 3,000 points per survey, but some are higher). You also get points for the profile surveys you do when you first sign up. They will get you your first couple thousand points. You can cash out to gift cards or a $25 paypal deposit (Takes 29,000 points). I have requested payment to my paypal account and their site says 2-6 weeks for processing, so as soon as I get that, I will add that to the list of proofs as well. They have a pretty steady flow of surveys everyday that come to my email, not that I qualify for them all, but there doesn't seem to be any shortage of possible surveys to take.
